Roman Catholic High School Transfer Student Admissions Application

Students presently enrolled in High School, desiring to TRANSFER to Roman Catholic High School must complete the following steps to be considered for admission: 
 
Step 1: Complete the entire application and submit through this web based form (NOTE: if you do not hit submit at the end your application it will not be processed)
 
Step 2: Submit copies of transcripts of all completed grades (8-11) and grades to-date for the current year, if the transfer is occurring during a school year. 
 
Step 3: Submit a letter of recommendation by a non-relative. 
 
Step 4: Arrange an interview with the Director of Admissions with the Admissions Committee.

Please note that false or misleading information presented on this application can lead to dismissal should the applicant become a student at Roman Catholic High School. Eligibility for participation in varsity sports at Roman Catholic High School may be contingent upon release by the principal of the present school. ​
Students seeking admission will be notified by mail of the decision of the Admissions Committee shortly after the admission interview. All imformation provided on this application will be held in confidence and will be reviewed only by the administration of Roman Catholic and the members of the Admissions Committee. If the student is not accepted, all date collected will be destroyed. 
 
Authorization for Release of Information 
Any statements misrepresented or omitted in this application may be cause for denial of this application or cause for dismissal. In submitting this application, both student and parents declare that all statements are true and correct. They also authorize Roman Catholic High School to investigate and contact relevant parties, concerning any matters the Admissions Committee feels are pertinent to the students' acceptance at Roman Catholic. Having read the above, I authorize the release of academic, discipline, counseling/guidance and health records to Roman Catholic High School, as they relate to this student. 

Admissions Essay

The student applicant is to write an essay on one of the following topics. The applicant can write on this form or attach his response separately to this application. 
 
Topic 1: The lesson we take from failure can be fundamental to later success. Recount an incident or time when you experienced failure. How did it affect you and what did you learn from the experience?

Topic 2: Discuss an accomplishment or event, formal or informal that will assist you with your future plans for life.
